MAX Membership Form - use this form to submit an application for membership in MAX, or to change any information you have on file with MAX.

JOIN - Become a MAX member by filling out our online membership application. You can pay your membership dues by credit card or check.

RENEW - Membership Renewal - renew your membership by updating your information and paying online with your credit card or sending a check to MAX.

Change of Address - to change your address that is on file with MAX, use the member update form or send an email to and include your old and new addresses.

The MAXfund Donation Form

The MAXfund welcomes donations of any amount. Your contribution will help MAX achieve its goal of creating a charitable fund. For more information, see The MAXFund information page. There is also a donation form that you can print and mail with your contribution.


The MAXfund Grant Application

Local non-profit organizations who wish to apply to The MAXfund for a financial gift may use this form to submit their application. CLICK HERE
